St. Petersburg Fire & Rescue has implemented a medical information program which we call the Telemedic Program. In the event of a true medical emergency, this program will assist the paramedics of St. Petersburg Fire & Rescue to quickly gather important medical data necessary to expedite your medical care and transport to the hospital. This program gives you the control to keep your medical information as updated as possible for just such an event.
The Telemedic forms will be kept in the red pouch which has magnetic strips on the back so that it can be placed on your refrigerator for easy visibility.
Please complete two forms per person just in case the paramedic needs to take one during a medical emergency. If you have a completed original DNR (DO NOT RESUSCITATE) form, please attach the original yellow copy to the Telemedic form.
