Benefits Division

The Benefits Division administers benefit programs for employees and retirees. This includes health, dental, life, EAP, 24-hour personal accident (AD&D) disability and accident care coverage. The staff acts as liaison with carriers for access to coverage issues. The staff is also responsible for processing actions.

CLASSIFIED EMPLOYEES

HEALTH INSURANCE

Several group health insurance programs are available to regular full-time non-sworn employees, temporary full-time employees hired for over six months, and their eligible dependents. The employee's share of the cost is 25% of the total premium. Eligible dependents include your legal spouse and unmarried children (natural, adopted or step-children) under the age of 25.

You may select one of the following types of plans available to suit your needs:

A Preferred Provider Organization (PPO) plan, which provides services through a provider network with coverage levels generally ranging from 80% to 100%. You also have freedom of choice in selecting services outside the network on a fee-for service basis with 30% co-insurance and deductibles.

A Health Maintenance Organization (HMO), which provides the services of physicians, hospitals and specialists associated with the plan for a total health care package. Only co-payments are required for medical services rendered, and claim forms are eliminated.

An Open Access Network plan (EPO), which provides for direct access services to physicians and specialists associated with the plan without a primary care physician referral.  

Your coverage will become effective the first of the month nearest 90 days of full-time employment. After initial eligibility, you may elect to change your health insurance coverage annually during the announced change period.

LIFE INSURANCE

$10,000 Term Life Insurance and Accidental Death and Dismemberment benefits are provided at no cost to regular full-time employees and temporary full-time employees hired over six months. $20,000 coverage is provided for employees in professional classifications. Additional supplemental term life insurance may be obtained at the employee's expense for the employee, spouse and children.

Your life insurance coverage becomes effective 90 days after full-time employment. After initial eligibility, you will be required to submit Evidence of Insurability to obtain the supplemental term life insurance coverage during the announced change period.

DENTAL INSURANCE

Dental insurance benefits are available to full-time employees, temporary full-time employees hired for over six months, and their eligible dependents at the employee's expense. Eligible dependents include your legal spouse, unmarried children (natural, adopted or step-children) under the age of 19, or, if enrolled as a full-time student at an accredited college, until age 25.

The prepaid dental HMO program provides services by a participating dentist associated with the plan. Many general and speciality services are provided for a small copayment. Other specialized services are provided at discounted rates. The PPO plan provides services through a provider network or outside the network on a fee-for-service basis.

Your coverage will become effective the first of the month nearest 90 days of full-time employment. After initial eligibility, you may elect to enroll or cancel the dental program annually during the announced change period.

FLEXIBLE SPENDING ACCOUNTS (FSA'S)

Flexible spending accounts allow employees to deduct funds for health and dependent (child care) from their paychecks costs on a pre-tax basis.

ACCIDENTAL DEATH INSURANCE

Optional Accidental Death and Dismemberment (AD&D) coverage is available at a nominal cost to regular full-time employees and temporary full-time employees hired for over six months. You may also elect coverage to include eligible family members. Your coverage will become effective the first month nearest 90 days of full-time employment. After initial eligibility, you may elect to enroll or cancel the AD&D coverage during the announced change period.

THE EMPLOYEE ASSISTANCE PROGRAM (EAP)

The EAP is a confidential, personal counseling and referral service for employees designed to assist them in resolving personal family and job-related problems that are having an undesirable impact on an individual's life, whether it be at work or at home.

Term Life Insurance and Accidental Death and Dismemberment benefits are provided to management employees at no cost. The amount of the benefit is approximately twice your annual salary. The maximum amount of term coverage is $100,000. Additional supplemental term life insurance may be obtained for the employee, spouse and children at the employee's expense.

Additionally, the management employees' life insurance program includes personal accident, short term disability and long term disability benefits.

Your life insurance coverage becomes effective the first day of full-time employment.
